  • Complete the form and mail it along with your check in the amount of your donation to:

  • Upon receipt of your donation, we will contact you with a form for completion of the wording you would like engraved on your leaf or stone.

The Memorial Tree

The Memorial Tree is a beautiful bronze sculpture of a large, lush oak tree branching with leaves, sitting upon a base of stones. The leaves and stones are inscribed with each contributor’s words of tribute and serve as a heartfelt presence of those who have significantly invested in the vision and mission of the Life Center.

A leaf recognizes a gift of $3,000 and a stone recognizes a gift of $10,000. Your generous gift will help support the value–based recreational, educational, physical fitness and sports activities for improving the quality of life for children and adults of all ages.
